Watch an OPD consultation
become a clinical note.
Press play on a recorded doctor-patient consultation in any of 10 Indian languages. You will hear the consultation as it was spoken, then watch AI MediScribe draft the note — complaints, history, examination, a provisional diagnosis with an ICD-10 code, the prescription, investigations and follow-up — and stop short of the one thing it must not do: invent a dose the doctor never said.
Pre-recorded role-play consultation. Processing is sped up for this demo. No real patient data appears on this page.

Five things worth watching for
The language stays as it was spoken
The doctor and patient switch between the local language and English mid-sentence. The draft comes out in English, structured.
It never guesses a dose
The doctor named three medicines without a dose or frequency. Every one of those cells says “confirm” — the draft asks the doctor instead of inventing a number.
It fills only empty fields
The vitals the nurse entered at triage were left exactly as they were. Apply never overwrites something already typed.
A test mentioned “only if needed” is kept separate
The conditional blood test is listed as discussed, not ordered. Only the CBC the doctor actually offered is ordered.
Nothing is saved until the doctor saves it
The replay ends at a filled form waiting for review. The doctor stays the author of the note.
The draft this consultation produces
Exactly what the doctor sees before applying it to the consultation form — written out here so you can read it without playing the demo.
Check before saving (4)
The draft opens with what it could not settle on its own.
- ‘Paracetamol’ — how often to take it was not said; fill in the frequency.
- ‘Cough syrup’ — how often to take it was not said; fill in the frequency.
- ‘Antihistamine’ — how often to take it was not said; fill in the frequency.
- ‘Paracetamol’ fits several medicines in your list (Paracetamol 500 mg, Paracetamol 650 mg) — pick the right one.

- 0:00 Doctor: Hello, come, have a seat. What's the problem?
- 0:02 Patient: Hello doctor, I've had a high fever for the last three days.
- 0:06 Doctor: Okay, when did this start?
- 0:07 Patient: It started on Friday. At first it was a mild fever, but then it slowly got worse.
- 0:14 Doctor: Okay. Any other symptoms — cough, cold, body ache?
- 0:20 Patient: Yes, a dry cough now and then, my whole body aches, and my head hurts too.
- 0:25 Doctor: Okay. Did you check your temperature?
- 0:28 Patient: Yes, when I checked it yesterday evening, it was showing 101.
- 0:32 Doctor: Okay. Any sore throat or difficulty breathing?
- 0:35 Patient: No.
- 0:36 Doctor: Okay. Are you taking any medicine right now?
- 0:38 Patient: Yes, I've been taking Paracetamol 500 mg, morning and evening, since yesterday morning.
- 0:42 Doctor: Okay. Any illness before this — sugar, blood pressure, asthma, anything like that?
- 0:49 Patient: No, I have no medical history.
- 0:51 Doctor: Okay. Any allergy to any medicine?
- 0:53 Patient: No, I don't have any allergies either.
- 0:56 Doctor: Okay, come, let me examine you. You do have a slight fever, and your throat looks red inside.
- 1:03 Patient: Doctor, it's nothing to worry about, is it?
- 1:05 Doctor: No, right now it looks like just a mild viral infection. Nothing to worry about. I'm writing you some medicines — Paracetamol, a cough syrup and an antihistamine.
- 1:16 Patient: Okay, doctor.
- 1:17 Doctor: And drink as much water as you can, get rest, and if your fever doesn't come down in two days, or the symptoms you have don't settle or get worse, do get a blood test done.
- 1:29 Patient: Okay, absolutely, doctor.
- 1:30 Doctor: Okay, I'll write all this down. And if you feel the need, you can get a CBC done even now.
- 1:36 Patient: Okay. Thank you so much, doctor.
- 1:38 Doctor: You're welcome. Anything you'd like to ask?
- 1:40 Patient: No doctor, just thank you so much.

From the consultation to the note, in four steps
- Step 1
The doctor consults normally
No dictation voice, no pauses, no keywords. The doctor and patient talk the way they already do — regional language, English, or both mixed in one sentence.
- Step 2
The system separates the voices
Speaker diarization splits the doctor's turns from the patient's, so the history is attributed to the patient and the plan to the doctor.
- Step 3
A structured draft is written
Complaints, history of present illness, past history, examination, a provisional diagnosis with an ICD-10 code, prescription, investigations, advice and follow-up — in English, from a consultation held in any of the supported languages.
- Step 4
The doctor reviews and saves
Anything the doctor did not say — a dose, a frequency, a duration — is marked to confirm rather than filled in. The note is saved only when the doctor saves it.
A scribe that only works in American English is no use in an Indian OPD
An Indian OPD doctor commonly sees 40 to 80 patients in a day, and in busy government hospitals more than that. The consultation is short, the room is noisy, and the language changes mid-sentence — a Hindi question, an English temperature reading, a Gujarati answer. The note still has to be written, and it is usually written after hours.
That is the consultation in this demo, not a clean studio dialogue. The doctor asks “Okay, when did this start?” in English inside a Hindi consultation. The patient reports “it was showing 101” in English. Medicines are named without doses, because that is how doctors talk when the dose is obvious to them. A scribe trained on Western English consultations degrades sharply on all three.
So the demo runs in 10 languages — Hindi, Gujarati, Marathi, Bengali, Tamil, Telugu, Kannada, Malayalam, Punjabi, Odia — on the same consultation, and you can read every transcript above. The draft comes out in English each time, structured into the fields a hospital actually bills and reports on.
What AI MediScribe will not do
Every one of these is visible in the demo above. They are design decisions, not gaps.
- It does not invent a dose, a frequency or a duration. If the doctor did not say it, the field says “confirm”.
- It does not overwrite anything already on the form. The triage vitals in this demo stay exactly as the nurse entered them.
- It does not order a test that was only discussed. The conditional blood test is listed as discussed, not ordered.
- It does not diagnose. The provisional diagnosis is the doctor's own words, coded to ICD-10 for billing and reporting.
- It does not save or sign anything. The doctor is the author of the note, start to finish.
AI Discharge Summary — demo in progress
The second demo takes a full inpatient episode — admission notes, daily rounds, investigations, procedures and medication charts — and drafts the discharge summary from it, the same way this one drafts an OPD note. It is being recorded now.
What people ask about AI medical scribes
What is an AI medical scribe?
An AI medical scribe listens to a doctor-patient consultation and writes the clinical note from it, instead of the doctor typing during or after the visit. AI MediScribe produces a structured OPD note — complaints, history, examination, provisional diagnosis with an ICD-10 code, prescription, investigations, advice and follow-up — which the doctor then reviews, corrects and saves.
Which Indian languages does AI MediScribe support?
You can play this demo in 10 languages today: Hindi, Gujarati, Marathi, Bengali, Tamil, Telugu, Kannada, Malayalam, Punjabi, Odia. Each one is the same consultation recorded in that language, with the transcript the system produced. The draft note comes out in English regardless of the language spoken.
Does it work when doctors mix English and Hindi in the same sentence?
Yes — that is the normal case in an Indian OPD, and this demo contains it. In the recording the doctor asks “Okay, when did this start?” in English inside a Hindi consultation, and the patient answers a temperature reading in English. The draft handles code-mixed speech without the doctor having to pick a language first.
How long does the draft take?
In real use the draft is usually ready within about a minute of the consultation ending. This demo compresses that wait to a few seconds so you are not watching a spinner, and says so on screen.
Does the AI decide the dose?
No. In this consultation the doctor names three medicines — Paracetamol, a cough syrup and an antihistamine — without stating a dose, frequency or duration. Every one of those cells comes back as “confirm”, and the draft opens with four items to check before saving. A scribe that fills in a plausible dose is a patient-safety problem, not a feature.
Can it overwrite what the doctor already typed?
No. Apply fills empty fields only. In the demo, the vitals the nurse entered at triage are on the form before the recording starts, and they are still unchanged after the draft is applied.
Where is patient data stored, and how does this sit with the DPDP Act?
Patient data is hosted in India and processed only under written contracts, in line with India's Digital Personal Data Protection Act, 2023. For hospitals that cannot send patient data off-site at all, the platform can be deployed on the hospital's own infrastructure.
Does it work with the HIS or EMR we already run?
AI MediScribe is part of One Learn AI's AI-native hospital information system, and it also runs alongside an existing HIS or EMR — the draft is applied into the consultation form you already use. Which route fits depends on your current system; that is the first thing we work out on a demo call.
Can it run without internet, on our own servers?
Yes. On-premise deployment is supported for hospitals — government, military and others — that cannot send patient data to a cloud service.
How much does an AI medical scribe cost in India?
One Learn AI prices per active doctor, per month, with volume slabs for hospital groups. The quote depends on bed count and consultant strength, so we share a specific number on the demo call rather than a list price here.
Is this a real patient?
No. The consultation is a scripted role-play recorded for this demo, and the patient on screen is “Demo Patient” with a demo UHID. No real patient data appears anywhere on this page.
How is this different from dictation or speech-to-text?
Dictation turns one person's speech into text that someone still has to structure. A scribe listens to an ordinary two-way consultation, works out who said what, and produces a structured note in clinical fields — including separating what was prescribed from what was only discussed.
